Position Summary: The On-Call RN works under the general direction of the Director of Nursing and serves as a member of the Interdisciplinary Team (IDT). Provides patient care consistent with the Plan of Care (POC) and is responsible for provision of services consistent with the mission, vision, and goals of Caring Hands Hospice (CHH), quality outcomes and regulatory standards
Qualifications:
● Graduate of an accredited school of professional nursing
● Current license as a Registered Nurse in the State of Louisiana with no restrictions
● Minimum of two (2) years full time experience as a Registered Nurse; Home care experience preferred
● Must be an employee of the hospice. If employed by more than one agency, he/she must inform all employers and coordinate duties to ensure quality service provision
Essential Functions/Responsibilities:
● Provides on-call coverage as scheduled and consistent with CHHs policies and procedures.
● Communicates with attending physicians, CHH staff members, and other agencies as necessary to coordinate optimal care and use of resources for patients/caregivers.
● Educates patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary.
● Reports changes in patients/caregivers status to IDT so that problems, plans, actions and goals are accurately and clearly documented and changes are reflected as they occur.
● Documents all information according to CHHs policies and procedures.
● Responds to medical or emotional crises and/or death of patients by visits to home/facility to provide direct care as appropriate.
● Responds to patient/caregiver phone calls in a timely manner consistent with CHHs policies and procedures.
● Communicates reports to the IDT following each shift.
● Informs the Access RN Clinical Manager and IDT or designee of unusual or potentially problematic patient/caregiver issues as identified.
● Assists RN Case Managers with their patient assignments by conducting scheduled after-hours visits as necessary to meet patient care needs.
● Documents work assignments thoroughly and completely in an accurate, prompt and neat manner including verbal and written communications as proscribed by CHH procedures.
● Consistently utilizes ipad device/tablet for point of care documentation.
● Reinforces care and teaching by making scheduled phone calls to patients following admission, for pain/symptom management and other identified needs.
● Maintains annual clinical competencies.
● Models prompt and regular attendance.
● Exhibits cost effective use of time, mileage and supplies.
● Demonstrates familiarity with and adheres to CHH policies and procedures, state, federal, and accrediting agency’s requirements.
● Provides notification of absence per CHH policy.
● Promotes and maintains respect for rights, privacy, and confidentiality of others. Respects patient and staff confidentiality at all times.
● Participates in committees, task forces and CHH events as requested.
● Attends staff meetings, mandatory in-services, and other meetings as appropriate or upon request.
● Assumes responsibility for own personal and professional growth including state licensing/certification requirements.
● Understands and exemplifies hospice philosophy, values, beliefs and team concepts in all job-related roles.
● Wears identification/name badge during working hours according to CHH policy.

Job Types: Part-time, PRN
Pay: $55.00 - $100.00 per day
Work Location: In person
