Our Award Winning Client is looking for a Primary Care Provider to provide comprehensive medical services to their participants. As a member of their interdisciplinary team, you will play a crucial role in ensuring the well-being of seniors in their community.
Essential Job Duties:
- Perform in-person comprehensive assessments, including initial, semi-annual, and unscheduled assessments.
- Collaborate with physicians and the interdisciplinary team to develop personalized care plans.
- Provide primary care and continuous medical coverage to participants.
- Assist in discharge planning from acute or long-term settings.
- Conduct regular visits to nursing homes to maintain participant care.
- Work collaboratively with physicians and community-based PACE physicians to ensure effective care coordination.
- Review outcomes and utilization data to establish best practices and policy improvements.
- Take part in rotating night and weekend call.
Skills and Technical Requirements:
- Graduation from an Accredited School of Nursing.
- Certificate as a Nurse Practitioner with Prescriptive Authority.
- NP State License.
- Minimum of two years of experience working with the senior population.
- Current knowledge of chronic care/geriatric issues and best practices.
- Additional technical skills as required.
