JOB TITLE: Residential Coordinator
IMMEDIATE SUPERVISOR: Program Director
Duties and Responsibilities:
The Residential Coordinator will oversee, direct and supervise the activities of all sponsored residential care workers in their performance of the following duties. Sponsored residential care worker having direct responsibility for guidance and supervision of the adults to whom he/she is assigned; oversee physical care; development of acceptable habits and attitudes; management of individual behavior and helping to meet the goals and objectives of any service plan; ensuring the daily needs of the individuals are properly met. Primarily interacts and communicates with the Program Director, Executive Program Director, Providers, Direct Support Professionals, Consultants, Psychologist, Individuals served, their families or other involved parties to insure the implementation of the individuals’ Person Center Plan. Performs required housekeeping duties in order to maintain a clean, safe and orderly environment; participates in staff meetings and in-service training as assigned; plans and implements special activities; completes special projects and other duties as assigned. This position requires the successful completion of training/certification in CPR, first aid, behavior management, medication administration. The successful completion of criminal history and central registry background investigations.
PRIMARY DUTIES:
- Develop and ensure on-going implementation of Individual Support Plans, Behavioral and Therapeutic Treatment Plans. Ensure adherence to Licensure, Human Rights, and/or other State, Federal, or Local regulations.
- Develop and coordinate meetings and training for staff, individuals and families regarding supports and services within the sponsored residential.
- Provide 24-hour on call accessibility for staff related to issues of individual support plans or advanced behavioral issues.
- Maintain current and accurate files on each person served.
- Coordinate and conduct intakes
- Input new individuals information in WAMs
- Processing of all files/ individual as well as provider.
- Have contact with families, guardians, CSB’, social workers etc.
- Coordinate and conduct monthly meetings and complete minutes.
- Provide input and recommendations (verbal and/or written) regarding issues of admissions, transfers, and discharges
- Review, receive and maintain the sponsor training folder
- Collect and review monthly documentation from providers
- Review support data and progress notes in order to modify/revise individual support plans, behavioral supports, treatment plans, etc. as needed.
- Perform other duties as assigned.
QUALIFICATIONS REQUIRED FOR THIS POSITION:
EDUCATION: Bachelor’s Degree preferred. Associates Degree accepted with experience in Human Service field, including but not limited to sociology, social work, special education, rehabilitation counseling, or psychology.
EXPERIENCE: Three year documented experience providing direct services (developing, conducting, and approving assessments and individual service plans) with adults with a diagnosis of an intellectual disability or other developmental disabilities One year experience interpreting and implementing Federal, state, or local regulations. One year of case management work.
SKILLS, KNOWLEDGE & ABILITIES:
- Comprehensive knowledge of intellectual disability or developmental disabilities, its causes and treatments.
- Comprehensive knowledge of licensure, human rights, and/or other regulations and the ability to apply these regulations.
- Ability to locate community and professional resources.
- Work independently, performing duties and responsibilities under general guidance. Communicate effectively, verbally and in writing.
- Possess computer skills and knowledge.
- Ability to analyze and summarize data into written reports.
- Develop support plans, assess and monitor progress.
- Knowledge of developmental training approaches.
- Ability to maintain effective inter and intra-agency working relationships.

PHYSICAL DEMAND:
The work is sedentary. Typically, the employee may sit comfortably to do the work. However, there may be some walking; standing; bending; carrying of light items such as papers, books, small parts, driving an automobile, etc. No special physical demands are required to perform the work.

3. Protecting individuals receiving services from abuse also includes using the minimum force necessary to restrain an individual.
4. Cooperates with any investigation, meeting, hearing, or appeal held under these regulations. Cooperation includes, but not limited to, giving statements or sworn testimony.
5. Perform any other duties required under these regulations.
6. Comply with HIPAA Privacy Rules and Regulations to protect individual privacy and confidentiality
7. Maintaining a good driving record and keep required trainings up to date
8. Maintaining accurate and complete records of individuals we service.
Job Type: Full-time
Pay: $38,000.00 - $42,000.00 per year
