This position is for a psychiatric outpatient medication management office that is part of a certified community behavioral health clinic that operates day shift Monday through Friday. Supervision experience required.
Summary:
Responsible for overseeing the nursing care and practice aspects of consumer care including monitoring medication management, policy, procedure, guideline application, and supervision of LPN’s Certified Medical Assistants in the Medical Services Division. Directs consumer care as indicated and necessary, to include medication administration, task prioritization, minor first aid.
Essential Duties and Responsibilities include the following. Other duties may be assigned to meet business needs.
- Oversee the nursing practices of LPNs in the Medical Services Division.
- Work with the Medical Services Division Director to ensure efficient communication throughout all areas of the Medical Services Division offices.
- Serve as the point of contact for Westbrook staff regarding medical concerns or questions of clients enrolled in the Medical Services Division program.
- Coordinate and communicate with onsite pharmacy, primary care and lab as needed for continuity of care.
- Monitor nursing tickets to make sure they are being addressed.

Obtain vitals of the clients as needed.

Complete prior authorizations when notified from pharmacy or client.

Complete urine drug screens as needed.

Administer scheduled injections to clients.

Complete incident reports when needed.

Responsible for onboarding, and call offs for Medical Services Nursing and CMA staff.

Responsible for providing feedback on Nurses and CMA’s performance to management staff responsible for their performance evaluations.

Attending or delegating a nurse to attend high risk meeting to provide coordination of care.

Work with the Director of Nursing to monitor, and train staff as needed, the policies, procedures, and operational guidelines of nursing services to ensure compliance.

Work with nursing staff at other non Westbrook facilities to ensure continuity of care is followed.

Work with nursing staff to maximize continuity of care and efficacy of nursing staff by making assignments for nursing staff, chart reviews, nursing meetings, and huddles.
- Ensure administration and compliance of medication management under the direction of the physician and as permitted by state regulations for each Medical Services consumer.

Ensure compliance with state code and standards of care.

Assist prescriber with virtual appointments as needed.
- Work with new and experienced staff regarding updates to policy, procedure, and training within medical services.
- Report communicable diseases to the Health Department and maintain communicable disease report binder.
- Assist prescribers and staff in navigating Board of Pharmacy, GeneSight, and Order Connect.

Serve as second level supervisor during staff counseling.
- Monitor Clozapine lab results and report accordingly for continuity of care.
- Responsible for Clozaril registration for prescribers.
- Monitor the general health condition of consumer, including vital signs, performing basic medical treatments, and dispensing medication.

Assist Division Director and Office Coordinator in maintaining fire safety for Medical Services ie. checking that all offices are empty during fire drills.
- Document all service activities including collateral contact in complete, accurate and timely manner according to Department of Health and Human Resources licensing regulation and Westbrook Health Services policy.

Responsible for providing orientation, monitor Relias and training to both new and experienced nursing staff as needed.

Comply with federal, state, and company policies, procedures, and regulations.

Maintains confidentiality of all information and complies with HIPAA regulations.

Must have a valid driver’s license and automobile insurance.
Education and/or Experience: Current LPN state licensure without restriction. At least one year of nursing experience. Prior computer/ PC experience using electronic medical record. Must pass criminal investigation background and DHHR background check as well as post offer drug screen.
