Clinical Manager Assistant (Home Health Utilization Management) - Registered Nurse (RN)* to join our San Diego team.

Qualifications:*
- Accredited bachelor’s degree in a clinical field. Nursing preferred.
- Professional licensure or registration (RN).
- Strong knowledge of Microsoft Office Systems and computerized systems
- Minimum of four years of clinical experience in home health care or equivalent
- Supervisory experience in home health care preferred.
- Working knowledge of State, Title 22, and Federal Medicare regulations
- Familiarity with community health principles and practices
- Ability to exercise independent judgment and work well under stress
- Excellent interpersonal and communication skills

What You Will Be Doing*
As the Home Health Utilization Management - Registered Nurse (RN), you will work closely with the Director of Patient Care Services and Clinical Field Staff to ensure the delivery of quality patient care and effective team performance, aligned with our company's philosophy, policies, mission, and vision. You will be responsible for coordinating and evaluating patient care services, ensuring compliance with local, state, and federal regulations.

Responsibilities:*
- Perform comprehensive assessments upon admission and determine appropriate clinical services needed.
- Coordinate care and evaluate the plan of care for patients.
- Assess visit frequencies based on patient acuity and plan of care.
- Ensure proper clinical coverage and interdisciplinary team communication.
- Review clinical records to maintain quality standards and adherence to care goals.
- Optimize staffing patterns and assign patients to clinicians based on territory.
- Collect and analyze staff visit data to ensure productivity standards are met.
- Facilitate interdisciplinary communication and provide training and education to team members.
- Stay updated on state and federal regulations and disseminate information to clinical staff.
- Collaborate with the Quality Assurance Department to implement new guidelines.
- Participate in company committees to achieve desired outcomes.
