Job Overview:
We are seeking a dedicated Counselor to join our team. The ideal candidate will have a passion for helping others and possess the necessary skills to provide effective counseling services.
Duties:
- Ensure access to necessary mental health and psychosocial services for referred residents.
- Conduct comprehensive psychological assessments to understand residents' needs and support their individual care plans.
- Identify and address current or potential adjustment difficulties.
- Address personal and interpersonal issues of residents within the nursing facility, focusing on emotional and social factors related to their medical condition or treatment.
- Provide short-term counseling, assess for suicidal ideation, and make referrals for psychiatric evaluation when necessary.
- Utilize casework and group interventions to resolve social and emotional challenges.
- Facilitate and lead group counseling sessions for residents.
- Collaborate with interdisciplinary teams and other professionals to enhance residents' well-being through coordinated care services.
- Offer consultations to the interdisciplinary team as required and maintain thorough documentation in electronic patient records.
- Participate in Quality Assurance programs within the facility.
- Coordinate support groups for staff and families as needed.
- Perform additional duties as assigned.
Qualifications:
- Preferred licensure as LCSW in the State of California. Master’s degree in a related field (MSW, MFT, Psychology) required, with 2-3 years of experience working with elderly adults and individuals with disabilities, ideally in long-term or healthcare settings, and possessing proficient social work skills.
- Proficiency in communication, assessment, and social work methods and techniques.
- Ability to facilitate support groups for residents, families, or staff as necessary.
- Exceptional interpersonal skills and a collaborative team-oriented approach.
- Ability to establish rapport quickly with colleagues, patients, and families.
- Strong time management abilities to effectively organize and prioritize tasks.
- Proficient in documenting and utilizing electronic health record software.
- Familiarity with medical terminology is advantageous.
Job Type: Part-time
Pay: $33.00 - $35.00 per hour
Expected hours: 32 per week
