Clinical Case Manager
DESCRIPTION:Wild Acre Inns began in 1972, providing state of the art residential treatment for individuals suffering from a broad spectrum of mental health diagnoses and disorders.
In 2014, the company was sold. Along with new ownership came a new name: Wild Acre and Mental Health Solutions. Wild Acre and Mental Health Solutionsprovides a continuum of care for individuals with persistent mental issues, including thought disorders such as schizophrenia and schizoaffective d/o, and brittle bipolar disorder. This care is provided in long term residential settings, which help to primarily supportresidents in managing tasks and activities of daily living. Wild Acre and Mental Health Solutions also serves young adults, with primary mood disorders, in a shorter term and more active residential setting. The newest program is designed to treat individuals with dual diagnosis to helpincrease their independence and to maintain sobriety.
Wild Acre and Mental Health Solutions also provides homecare care services for individuals who require various levels of oversight and support to safely remain living in their home, or in preparation of a step down from a more structured environment to a more independent living environment.
The Clinical Case Manager is responsible for providing comprehensive case management for individuals with persistent mental illness. The Clinical Case Manager will work closely with the Program Director and Assistant Director in the formulation of treatment planning, responding to crisis within the milieu, and planning for the needs of residents to ensure efficacy of service delivery.
REPORTS TO: Program Director
SALARY: $50,000-$54,000
DUTIES AND RESPONSIBILITIES:
- Provide case management services as part of a multidisciplinary team
- Develop treatment plans with residents related to mental health needs
- Familiarity and liaison with community resources
- Participate in daily meetings/community groups as defined by the program
- Document daily progress notes for all residents on case load
- Act as a primary case manager to up to 4 residents
- Participate in completing intake assessments, monthly treatment meetings, and aftercare planning
- Collaborate with outside treatment teams and families
- Medication management and administration
- Assist with structuring time in the community that may include recreational, educational and/or vocational goals
REQUIREMENTS:
Requires a MA/MSW in related field with preferably one year of experience working with persistent mental illness.Ability to run small groups. Computer literacy and skill in the use of computers and related software applications.Ability to consistently meet all necessary background and credentialing requirements. Must have valid driver’s license and reliable transportation to and from work and for the transportation of clients as needed.
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
