Assists the Health Home Nurse Care Manager to support service users in the development and maintenance of healthy lifestyles.
Job Duties:
1. Reviews updates at the beginning of work shift by: a) emptying and reading all correspondence from personal mailbox; b) reading and responding to email; c) listening and responding to voicemail; d) reading Anchor Update Report.
2. Establishes a professional, caring relationship with all people being supported by the team. Has awareness of and is sensitive to each person's (and of co-worker’s) cultural, racial, and ethnic background. Develops and maintains relationships with hospitals and medical providers in the community to offer comprehensive services to service users.
3. Compares ongoing outcome data to baseline data to monitor each service user’s progress. Schedules contacts with service users. Assists to schedule medical appointments with external resources and follows up with service users. Assists in obtaining service users’ health records, labs, vitals, etc.
4. Communicates with Archway staff and all other disciplines on an ongoing basis to ensure collaboration of care. Reviews results of laboratory tests and other diagnostic procedures. Identifies significant clinical findings, makes conclusions and intervenes appropriately. Supports and monitors compliance of the individual’s self-administration of prescribed medication, including checking of prescription labels and storage. Administers injectable medication as needed. Interprets the appropriate information needed to identify each service user’s requirements relative to age-specific needs.
5. Stays up-to-date on somatic health issues and works with the nursing team to research and provide appropriate resources. Works as a team with other nursing staff to deliver quality services to focus on the improvement of the somatic health of people using the service, prioritizing prevention and education. Focuses on program goals specific to reducing emergency room visits and hospitalizations related to somatic health issues.
6. Monitors and maintains quality indicators and disease management indicators surrounding comorbid chronic conditions and healthcare screening, as indicated. Monitors and maintains appropriate reporting mechanisms, such as statistical data, as indicated.
7. Assists in providing leadership to the implementation and coordination of activities. Assists in training nursing staff. Assists in directing the activities necessary to obtain and maintain State of Maryland standards and CARF accreditation. Assists in maintaining compliance with all applicable licensures, certifications, and/or accreditation requirements and agency policies and procedures.
8. Provides training in areas including, but not limited to, communicable disease prevention as needed. Helps to promote services by attending community events and visiting other agencies and businesses in the community.
9. Focuses on meeting all COMAR regulations, CARF standards, and HIPAA laws. Attends certification/recertification training as scheduled, including maintaining licensure requirements as set forth by the Maryland Board of Nursing. Completes all other training as scheduled/assigned.
10. Performs other tasks related to the overall scope of the job as assigned.

Qualifications for this Position:
1. Licensed as a Registered Nurse with the Maryland Board of Nursing.
· Minimum of an Associate’s degree allows for annual education stipend.
2. Strong interpersonal skills.
3. Good documentation skills. Proficient in Microsoft Office; especially Word, Excel, and PowerPoint.

Pay: $29.48 - $35.51 per hour
