**Job Summary:**
We are seeking a dedicated Registered Nurse with exceptional knowledge of home health, cath lab and wound care to join our growing team. The ideal candidate will be responsible for performing medical screenings and providing care to clients. Experience with medical terminology, IV infusion, and administering injections is highly desirable.
**Duties:**
· Completes initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients/clients.
· Regularly re-evaluates the patient's/client’s nursing needs and evaluates the outcomes of care.
· Monitor, record, and communicate changes in patient's condition and respond as appropriate to the needs of the patient and family.
· Develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.
· Maintain accurate patient records and documentation
· Administer medications and treatments as prescribed by physicians
· Prepares clinical and progress notes. Completes appropriate documentation in a timely manner.
· Demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.
· Supervises, LPNs/LVNs and paraprofessionals providing services to patients/clients.
· Conduct aide competency evaluations if qualified with two years of clinical experience and one year of home health experience.
· Educate patients and their families on health management. Teaches and provides clinical direction to other nursing personnel. Assigns home health aides to specific patients
· Performs on-call responsibilities and provides on-call services to patients/clients and their families as assigned
**Skills:**
The successful candidate should possess the following skills:
· Knowledge of medical terminology and clinical documentation for home health patients
· Experience in home health setting, wound care, administering injections and IV infusion
· Ability to provide care in patient’s home, and long-term care settings
· Strong communication and interpersonal skills for patient interaction
· Collaboration with doctors and other healthcare professionals

Pay: $38.67 - $42.50 per hour
