We are seeking a professional to join our new Enhanced Care Management (ECM) program, driving strategic innovations that impact health outcomes and community stability through the lens of Social Drivers of Health (SDOH).

This position offers a competitive hourly salary ranging from $25.00 to $28.00/hour, depending on experience. It is a full-time, non-exempt role.
POSITION DESCRIPTION
This role is very specific for the Enhanced Care Management (ECM) (CalAIM) program for the State of California. A qualified candidate should have prior knowledge and work experience with ECM. Under the general direction of the ECM Program Manager this position is responsible for working effectively with, and as part of CalAIM to support ECM Members in improving their whole health, through outreach and engagement activities, which are primarily field based. This position works collaboratively as part of the ECM team, members and families, and other professionals. Community Health Workers (CHW) may assist with a variety of concerns impacting individuals and families, including the control and prevention of chronic conditions or infectious diseases, behavioral health conditions, and need for preventive services. Additionally, CHW services can help patients receive appropriate services related to perinatal care, preventive care, sexual and reproductive health, environmental and climate-sensitive health issues, oral health, aging, injury, and domestic violence and other violence prevention services.
Community Health Workers provide a broad range of services focused on improving the lives of individuals, families, and communities through providing meaningful, culturally relevant connections to healthcare and healthcare related services, which address the Social Determinants of Health (SDOH). CHWs service is informed through the lived experience of the CHW and their families, social circles, and communities.
MAJOR DUTIES AND RESPONSIBILITES
- Responsible for assisting Members in navigating the healthcare system by finding and following up by phone and in person with eligible ECM Program (ECM) Members, helping Members successfully participate in their medical and/or behavioral health care by overcoming barriers to care, and sharing information on barriers to improve care and outcomes.
- Model the highest ethical behavior in relationships with co-workers, supervisors, Members, Providers, and colleagues in the community.
- Responsible for engaging with Members, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as Motivational Interviewing) that promotes collaboration between the Member and his or her medical/behavioral team, as well as to increase the Member’s sense of control over their whole health.
- Delivering information about health and wellness in ways that the community can easily understand and providing information on ECM Member benefits and services.
- Providing advocacy on behalf of ECM Members in the home, the community, and in provider organizations.
- Responsible for building and maintaining a positive working relationship with Providers, including, but not limited to, interactions within Provider offices, communication over the phone, and through digital means such as email and fax.
- Responsible for promoting a collaborative and effective working environment within ECM by engaging in evidenced-based communication strategies (such as Motivational Interviewing) when discussing responsibility/sharing of tasks, effectively resolving conflicts as they arise, and collaborating on Member case discussions.
- Collaborates on Member care issues with other teams, participating in weekly and ad hoc case reviews, and consults with RN Lead Care Manager and/or the Behavioral Health Care Specialist before taking any action that is clinical in nature.
- Model commitment to continuous quality improvement by engaging in quality improvement initiatives and projects, such as identifying and addressing gaps, and by identifying, developing, and testing new practices for improving the outcomes of the ECM.
- Ensures documentations is accurate, useful and in compliance with regulatory requirements and accreditation standards.
- Participates in all appropriate staff meetings or other activities as needed.
- Help facilitate interactions with the ECM team members.
- Ensures the privacy and security of PHI (Protected Health Information) as outlined in Inland Housing Solutions policies and procedures relating to HIPAA compliance including attending annual compliance training.
- Responsible for assisting Members in navigating the healthcare system.
- Perform other duties as needed.
QUALIFICATIONS FOR THE JOB
Minimum Requirements:
- High School Diploma or equivalent.
- Completion of Community Health Worker Certification course.
- Minimum of 1 year ECM experience.
Required License, Certification, Association:
- Valid driver’s license with good driving record and be able to drive within applicable locality with reliable transportation.
