Fishkill Home Care, a premier home health care agency serving the residents of Dutchess County, is looking for an experienced and motivated RN that is ready to oversee our clinical and day to day operations as The Administrator/Director of Patient Services. The ideal candidate will have substantial experience with Certified Licensed Home Care Agency and possess a significant amount of formal supervisory experience in home health Care.
Ø We are paying a $3,000.00 signing bonus for this position (to be paid upon successful completion of 90 day probationary period).
Responsibilities include the following:
- Responsible for Supervision and oversight of LPNs,RN’s and the PCA staff in the field and at client’s homes creating a positive team environment.
- Plan, direct, coordinate and monitor the delivery of all services to the agency’s home care clients
- Maintain MD orders and nurse visit schedule per DOH regulations
- Complete in home assessments
- Development and maintenance of client care plan
- Supervises the effectiveness and compliance of the agencies nursing team
- Revises agency policies and procedures as necessary,
- Responsible for directing the agencies QA/PI Program
- Responsible for the overall coordination of the clinical operations for the provision of quality care services for the Company
- Supervises & evaluates the nursing team regarding interpretation, implementation & compliance with Company policies, state and federal regulations and Joint Commission standards.
- Assist in the planning and development of agency structure, strategic planning, clinical and administrative policies and procedures
- Responsible for agency quality reporting, developing organizational policy and procedures, and assuring compliance
- Identify admission criteria for new clients and evaluate eligibility for home health care benefits
- Travel as required for patient care.
Requirements:
- 2 years of experience working within the home providing direct patient care
- Bachelor’s of Science in Nursing is highly preferred, NY RN License
- Skilled in areas of staff education and development,
- Computer literacy is a must- complete computer documentation
