Our hospice agency is looking a full-time case manager.
This job entails direct palliative care to terminally ill patients and their families while functioning within a medically approved plan of care. The Hospice case manager will deliver services under the direction of the interdisciplinary team and the Director of Clinical Services. This job also involves obtaining medical records for patients.
Job Duties
. Making regular visits to patient homes to assess patient's physical, emotional, spiritual, and social needs.
· Follows Individualized Plan of Care established for each patient.
· Gathers and reports care data and implements care accordingly.
· Provides care under the supervision of a Registered Nurse/Doctor.
· Immediately report any significant change to the Nurse or Physician.
· Adheres to patient rights, abuse reporting, and confidentiality policies.
· Forster’s and maintains an effective working relationship with peers, professional staff, and other personnel.
· Supervises and delegates duties to the Certified Nurse Assistants on shift.
· Able to complete appropriate documentation in an efficient and timely manner.
· Demonstrates good judgment with proper decision-making abilities.
· Participates in the interdisciplinary team meetings and other performance improvement activities.
· Provides intervention support as needed related to terminal illness and loss.
· Provides emotional and spiritual support and makes appropriate referrals to other team members.
. Entering medications orders and reviewing them for patients
Requirements
· Must be currently licensed as a Licensed Practical Nurse by the Nevada State Board.
· Prefer one (1) year of experience as a Licensed Practical Nurse in a hospice setting.
· Successful completion of Background check pursuant to NRS 449.188 and NRS 179A.100.
· Read, speak, write, and understand the English Language.
· CPR/First Aid Certified
· Must be able to handle frequent interruptions.
· Must be able to cope with emotionally charged situations encompassing, patients, families, and other caregivers.
· Reliable transportation
· Active Driver’s license and vehicle insurance
