The Westbrook Youth Recovery Center case manager provides case management services to include: assessment, service planning and service plan evaluation, linkage/referral, advocacy, crisis response planning, travel, and reporting. This position provides opportunities to work with youth and families to help make changes in the lives of young people and empower them to take control of their lives and become the person they want to be. The Youth Services Case Manager will work directly with students in secondary education, ages 14-20.
Essential Duties and Responsibilities include the following. Other duties may be assigned to meet business needs.

Implements programs and activities to reduce criminal behavior.

Maintains high visibility throughout the building and grounds.

Collaborates closely with the education coordinator as needed.

Monitors student behavior.

Maintain an awareness of available community resources.

Acts as a liaison between family and other treatment staff as needed.

Communicates effectively with students, parents, administration, and staff.

Attends scheduled Leadership Team Meetings and IEP meetings as needed/necessary.

Participates in professional growth opportunities.

Ensures timely completion of all client documentation and assessments

Makes recommendations to staff utilizing needs assessment data and observations.

Maintains confidential participant records and/or data. Prepare written internal and external reports in a timely manner.

Some travel between sites and school required.

Must have a valid driver’s license and automobile Insurance.

Must successfully complete program specific required training.
Education and/or Experience: Bachelors degree in Human Services discipline (Psychology, Criminal Justice, Regents with health specialization, Recreational therapy, Political Science, Nursing, Sociology, Social Work, Counseling, Teacher education, or Liberal Arts) or a Bachelor’s degree in another discipline with a minimum of 12 credit hours in the human services disciplines listed previously. Prior computer/PC experience using computerized Medical Records and scheduling systems is preferred, but not required. Incumbent must also possess basic math skills. Must pass Criminal Investigation Background and DHHR background check as well as post offer drug screen.
Physical Requirements:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The supervision and treatment of youth requires prolonged periods of walking and standing and physical agility. May be required to run and climb flights of stairs. Must be able to participate in physical interventions with youth when necessary. While performing the duties of this position, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. May occasionally have to move or lift 25+ pounds Will be required to complete Right Response and CPR training.
