We are seeking a dedicated and compassionate Field Registered Nurse (RN) to join our team at HomeCaire, a leading personal care agency. As a Field RN, you will be responsible for completing initial assessments and developing individualized plans of care for our patients. Additionally, you will conduct re-assessments for existing patients to ensure their ongoing care needs are met.
Responsibilities:
1. Conduct initial assessments of new patients to determine their healthcare needs, including physical, emotional, and social factors.
2. Develop comprehensive individualized plans of care based on assessment findings, patient goals, and physician recommendations.
3. Complete re-assessments for existing patients at designated intervals to evaluate their progress and update their care plans as needed.
4. The ability to work quickly and efficiently to ensure assesments are completed timely.
5. Collaborate with interdisciplinary teams, to coordinate and implement patient care plans effectively.
6. Provide patient education on treatment plans, medications, and self-care techniques to promote independence and improve outcomes.
7. Maintain accurate and timely documentation of patient assessments, care plans, and interventions in our Electronic Medical Records (EMR) system.
8. Monitor and evaluate patient outcomes, documenting changes in condition and reporting concerns to the appropriate providers.
9. Participate in ongoing training and professional development opportunities to enhance your skills and stay updated on best practices.
Qualifications:
1. Valid RN license in the state of Ohio.
2. Minimum of 2 years of clinical experience as an RN, preferably in home health or a related setting.
3. Strong assessment and critical thinking skills with the ability to develop individualized care plans.
4. Excellent communication and interpersonal skills to collaborate effectively with patients, families, and administrative staff.
5. Proficiency in using electronic medical records (EMR) systems for documentation and data entry.
6. Ability to work independently and prioritize tasks in a fast-paced environment.
7. CPR certification and other relevant certifications may be required.
8. Reliable transportation and a valid driver's license for travel to patient homes.
