SUMMARY:
The BH Crisis Care Manager is responsible for coordinating, screening, and providing crisis intervention to AMC patients who are identified to need behavioral health services. Provide crisis intervention, identification of patients at high risk, coordinate services with BH therapists/psychiatric providers, and help identify additional resources needed by patients. Collaborates with medical staff, BH therapists, and Psychiatric providers to enhance the integration of behavioral health and primary care of patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Identify, track, and manage care for Behavioral Health high risk patients.
- Conducts comprehensive SI/HI assessments with patients at our school-based health centers/clinics and completes process for hospitalization as needed.
- Conduct initial care coordination screening eligibility.
- Identify psychological, emotional and behavioral issues based on information obtained from initial intake or crisis intervention.
- Provides follow ups with patients to increase engagement in BH services and treatment completion.
- Provides ongoing clinical assessments, monitoring and follow up for high-risk patients.
- Focus on using behavioral patient activation interventions, including motivational interviewing and self-management support.
- Assess psycho-social needs and provide community resources and referrals to outside agencies/programs when appropriate.
- Promotes clear communication amongst care team, including medical staff and BH/PSYCH staff regarding patient care plans.
- Participates in patient staffing and care conferences, and in the development and implementation of multi-disciplinary approaches to patient care.
- Contacts and develops community resources on behalf of patients and the Center and acts as patient advocate as appropriate.
- Documents case management services through recording in the patient’s medical record and completion of reports and statistics.
- Handles protective health information in a manner consistent with the Health Insurance Portability and Accountability Act (HIPPA).
- Represents Alivio Medical Center in professional and community organizations as assigned
- Participates in staff meetings and other meetings as assigned.
- Performs other job-related duties as assigned.
This position requires compliance with all of Alivio’s written standards, including its Standards of Conduct, Joint Commission standards, all policies and procedures and Corporate Compliance requirements. Compliance will be considered as part of the regular performance evaluation.

WORKING CONDITIONS:
OSHA Category 3 – Involves no regular exposure to blood, body fluids, or tissues, and tasks that involve exposure to blood, body fluids, or tissues are not a condition of employment
QUALIFICATIONS:
Education:
- Master’s Degree in Social Work, Counseling or related field from an accredited school. A concentration in clinical services is desired.
Certification or Licensure:
- Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) preferred.
Experience:
Demonstrated Competencies:
- Demonstrated acceptable skill level in mental health assessment and case management. Experience in crisis intervention. Demonstrated ability to develop and maintain good working relationships with patients, families and professional colleagues. Experience in interdisciplinary team approach to patient care is desired. Flexibility and ability to cope with stress. Adherence to professional ethics and standards consistent with the National Association of Social Workers, the American Counseling Association or Society for Clinical Social Work.
