What You’ll Be Doing:
- Care Plan Oversight: Overseeing the development and implementation of Individual Care/Health Action Plans for each client.
- Flexible Service Delivery: Offering services in a variety of settings, including office-based, telehealth, or community-based services, based on client convenience.
- Outreach and Engagement: Conducting activities to facilitate linkage to the ECM program, including phone calls, mailed information, and community visits.
- Needs Assessment: Conducting individual assessments to understand client needs and disseminating information accordingly.
- Service Coordination: Coordinating the delivery of services from various social service providers to program participants.
- Medical Coordination: Coordinating all medical appointments and maintaining updated medical files.
- Transport Logistics: Arranging transportation for clients as required.
- Legal and Probation Support: Assisting program participants with Probation and Parole requirements, including providing progress reports and supportive documentation.
- Client Advocacy: Coordinating and advocating on behalf of clients with healthcare professionals, with the client's permission.
- Community Connection: Connecting clients to essential medical services, social services and support groups as needed.
- Client Support: Accompanying clients to office visits as needed and appropriate.
- Progress Evaluation: Regularly evaluating client progress and updating goals as necessary.
- Mental Health Promotion: Actively promoting mental health and wellbeing among clients.
- Timely Documentation: Completing all necessary documentation within established timeframes according to individual action plans.
- Team Collaboration: Attending weekly staff/team meetings and participating in supervision sessions.
- Continuous Learning: Engaging in assigned training sessions, such as Motivational Interviewing and ACEs Certification.
- Data Management: Collecting and entering client data as needed. Keeping EHR/EMR
- Positive Environment Cultivation: Maintaining a positive, life-giving environment for clients and keeping client files updated with all relevant medical, legal, and social service documents.

Pay: $54,000.00 - $60,000.00 per year
