General Job Duties
Under the supervision of the Clinical Program Director, the case manager will assume the following responsibilities:
Duties:
- Conduct assessments to identify clients' needs and develop individualized service plans
- Provide crisis intervention and support to clients in emergency situations
- Collaborate with community resources and agencies to coordinate services for clients
- Monitor and evaluate clients' progress towards goals and make necessary adjustments to service plans
- Document all client interactions, services provided, and progress towards goals
- Advocate for clients' rights and access to appropriate services
- Provide mentoring and guidance to clients on various life skills
-Participate on the intake team and engage in the decision-making process.
-Evaluate goals for placement in cooperation with the D.C.F.S. worker.
-Manage and supervise caseload of children in placement at the group home or Transitional Living Program and ILO.
-Participate in Administrative Case Reviews (ACRs), attend court hearings, and maintain liaison with D.C.F.S. staff.
-Assist in screening natural/foster parents to assure that they meet visitation requirements, minimize exposure to potential abusive experience to the child, and prepare progress reports on the child and biological family members.
-Perform other administrative duties as assigned by the Clinical Program Director, including, but not limited to, supervising Child Welfare Workers.
-Coordinate with community agencies to carry out the treatment plan.
Qualifications:
- Bachelor's degree in Social Work, Psychology, Counseling, or related field (Master's degree preferred)
- Previous experience in case management, social work, or related field
- Knowledge of Medicare, child welfare, and behavioral health systems
- Experience in crisis management, childcare, motivational interviewing, and supervising
- Strong interpersonal skills and ability to work with diverse populations
- Ability to handle crisis situations with empathy and professionalism
- Excellent organizational skills and attention to detail
-CWEL recommended, though not mandatory that training will be provided

Pay: $25.00 - $27.14 per hour
