Job Description:
We are seeking a highly skilled and motivated Contract Physical Therapist to join our team for a 13-week assignment in a Home Health setting. As a Contract Physical Therapist, you will be responsible for providing exceptional physical therapy services to patients in the comfort of their own homes.
Responsibilities:
- Conduct comprehensive evaluations of patients' physical conditions to determine their needs and develop personalized treatment plans.
- Implement evidence-based physical therapy interventions aimed at improving patients' mobility, strength, flexibility, and overall function.
- Provide education to patients and their caregivers on proper techniques for exercises, activities of daily living, and injury prevention.
- Monitor patients' progress closely and adjust treatment plans as necessary to achieve optimal outcomes.
- Communicate effectively with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated care and maximize patient outcomes.
- Maintain accurate and up-to-date documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in accordance with regulatory standards and organizational policies.
- Adhere to ethical and professional standards of practice, demonstrating compassion, empathy, and respect for patients' dignity and confidentiality.
- Participate in interdisciplinary team meetings and collaborate with colleagues to enhance patient care and achieve organizational goals.
Requirements:
- Graduate of an accredited Physical Therapy program with a valid license to practice as a Physical Therapist in the state of Delaware.
- Minimum of 1 years of experience working as a Physical Therapist, preferably in a Home Health or similar setting.
- Strong clinical skills and proficiency in performing comprehensive physical therapy evaluations and developing individualized treatment plans.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients and effectively collaborate with other members of the healthcare team.
- Demonstrated ability to work independently and manage a caseload of [X] patients per day, prioritizing tasks and meeting deadlines.
- Flexibility and adaptability to navigate various home environments and accommodate patients' unique needs and preferences.
- Current CPR certification and compliance with state licensure requirements and professional regulations.
- Valid driver's license and reliable transportation for travel between patient homes.
- Commitment to maintaining continuing education and staying abreast of developments in the field of physical therapy.
Job Types: Full-time, Contract
Pay: $50.00 - $55.00 per hour
