Join our client's team as a passionate and dedicated Supervisor of Clinical Services, where you'll play a pivotal role in coordinating comprehensive clinical care for hospice patients. Reporting to the Program Director, you will lead the day-to-day operations, ensuring that all services align with our organization’s mission and comply with relevant regulations.

Requirements:
1. Licensed Registered Nurse in Connecticut.
2. One of the following:
- Master’s degree from an approved program with at least one year of clinical experience in home health or related community health.
- Bachelor’s degree in nursing with three years of clinical experience, including at least one year in home health or community health.
- Registered Nurse with continuous supervisor experience in a home health agency in Connecticut since January 1, 1979.
- Diploma or associate degree in nursing with three years of clinical experience within the past five years, including at least one year in home health or community health, and certification by the American Nurses’ Association or completion of relevant coursework.
Job Duties:
- Coordinate all clinical services, including planning, organizing, directing, and supervising.
- Ensure adherence to patient care rules, regulations, standards, policies, and guidelines.
- Implement and oversee the IDG Plan of Care and ensure high standards of hospice care and symptom management.
- Monitor and evaluate patient care activities to maintain compassionate and competent care.
- Collaborate with the Program Director on regulatory compliance and communication.
- Supervise and support program staff, participating in hiring, training, and development.
- Conduct home visits to assess care and provide guidance to staff.
- Foster effective communication and relationships with hospice and inpatient facilities.
Functions:
- Direct day-to-day healthcare operations to uphold organizational standards and goals.
- Ensure quality and continuity of care, including 24-hour service availability.
- Develop and implement policies, procedures, and staff training programs.
- Participate in interdisciplinary team meetings and program development activities.
- Promote a positive work environment, encouraging team collaboration and continuous improvement.
- Support the hospice's Quality Assessment and Performance Improvement Program (QAPI).
- Adhere to state, federal, and Joint Commission standards.
Working Conditions:
- Perform heavy work, including occasional lifting and frequent physical activities within patient homes.
- Navigate various environmental and safety conditions.
Documentation:
- Maintain ethical standards and comply with professional nursing practices.
- Ensure accurate and timely completion of reports and paperwork.
