We are looking for a qualified LPN to promote and restore patients’ health. You will work with a variety of patients and you will provide direct nursing care along with physical or psychological support when required.
Qualifications:
- 1 year(s) of experience in long-term care preferred
- Experience with Sigma EMR a plus
- Must possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in this state.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to interact professionally with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Must possess leadership and supervisory ability and the willingness to work effectively with and supervise other personnel.
- Must be willing to seek out new methods and principles and incorporate them into existing nursing practices.
- Must be able to relate information concerning a resident's condition.
- Must be able to cope with the mental and emotional stress of the position.
- Must possess at a minimum basic computer skills.
- Must possess in-depth and comprehensive clinical and documentation skills
Responsibilities include, but are not limited to the following:
- Determines required quality of care by developing and interpreting the facility’s policies and standards of care; enforcing adherence to state board of nursing and state nurse practice act requirements and to NYSDOH regulations; making or recommending adjustments to protocol /policy as needed.
- Completes frequent rounds to all resident care areas during the assigned shift to ensure provision of appropriate resident care
- Provides daily shift change report to CNA staff and shift to shift report with incoming / outgoing nurses
- Completes narcotic count with incoming / outgoing nurse.
- Ensures all residents have appropriate armbands in place.
- Ensures completion of resident weights and vital signs as required
- Ensures ordered labs and diagnostics are completed and documented in Sigma. Any resident refusals are documented as well
- Reports any changes in resident condition to the RN / charge nurse / supervisor
- Ensures documentation of transfers / OOP / clinic appointments / dialysis in Sigma
- Manages Sigma dashboard, ensuring documentation of medication / treatment administration is completed by the end of the shift
- Ensures all medication and vaccine administration is documented. Any medication not available or awaiting delivery should be put on hold.
- Ensures all topical treatments, trach care, suctioning, GT / PEG care are administered and documented in Sigma
- All catheter care is documented in Sigma
- All IV care / provision, calorie counts, fluid provision is documented in Sigma
- Completes 24 hour report, episodic CCPs and daily progress notes in Sigma
- Completes progress notes (and 24 hr report) as needed.
- Ensures good customer relations by maintaining ongoing communication with residents and families by answering questions and providing information as needed
- Maintains communication with Physicians, provides clinical information and obtains telephone orders for resident management as needed.
- Maintains communication with Administration, providing information regarding issues / potential issues within the facility
- Participates in Survey preparation as instructed by Administration
