GENERAL STATEMENT OF RESPONSIBILITES:
This position is responsible for the day-to-day needs of each individual receiving service under the license of Essential Family Services LLC. The position requires knowledge of DBHDS Guidelines and Regulations and the Office of Human Rights Regulations. Additionally, other responsibilities and duties include implementing individual service plans, data collection, good oral and written communication skills; experience in behavior management, and individual programming for various levels of individuals who have an intellectual disability. The qualified applicant will have experience working with a variety of individuals with various service needs. The Program Manager will supervise this position.
MAJOR DUTIES:
Provide training, supervision, and support for individuals receiving services.
Adhere to State licensure and human rights regulations and guidelines.
Maintain effective quality services and appropriate staffing for individual or individuals receiving services.
Maintain current, accurate, and appropriate individual records and other relevant documentation involving individuals and programming.
Provide proper behavioral assistance and intervention for individuals.
Treat all individuals receiving services with dignity and respect at all times.
Provide input regarding the provision of services for individuals receiving services.
Provide In-Home Services according to the individual’s (ISP) needs and schedule.
Transport individuals receiving services to various community sites.
Maintain and renew all First Aid and CPR training and other required certifications necessary to carry out program services.
Maintain an environment that implements quality health and safety management standards to protect individuals' health, safety, and welfare.
Adhere to all Essential Family Services LLC program policies and procedures.
Maintain knowledge of current services by attending workshops, training, and conferences.
Maintain confidentiality and security of individual service records.
QUALIFICATIONS:
High School Diploma or GED.
Valid operator’s driving license
Valid driving record
Current TB Test
Satisfactory Criminal Records Report and Background Check

DBHDS and Human Rights Licensure Policy and Procedures
SKILLS IN:
Supervising individuals with a wide variety of service needs.
Implementing individual Service Plans and behavioral intervention plans.
Implementing program services with outside agencies, organizations, and professionals.
Maintaining professional, ethical standards.
Satisfactorily complete all required training in First Aid, CPR, & Therapeutic Options (TOVA)
Job Types: Full-time, Part-time
Pay: $15.00 - $17.90 per hour

Education:
