Job Summary:
We are seeking a highly skilled and motivated physical therapist with experience in providing physical therapy services in patients’ homes. The Physical Therapist will be responsible for evaluating patients, creating individualized treatment plans, and teaching patients physical therapy exercises for healing and rehabilitation. The successful candidate will have a passion for helping people achieve wellness through physical therapy treatment.
Job Details:
- Homecare visits in the clients most functional environment
- 40 visit/week requirement for benefits
- Visits are 30 minute treatments
- PM Shift (25 visits from 3:00-8:00pm), AM Shift (15 visits any time
Responsibilities:
- Provide efficient, high-quality physical therapy treatment to patients and caregivers in the home setting
- Document patient progress, reassessments, and outcomes using electronic medical records.
- Educate and instruct patients, caregivers, and families on therapy treatment plans, goals, and exercise programs.
- Monitor patients’ progress and adjust treatment plans as necessary to ensure optimal results.
- Participate in ongoing training, professional development, and continuing education opportunities to stay current with industry standards and advancements.
Requirements:
- Graduate of an accredited Physical Therapy program and licensed to practice in New York state.
- Minimum of two years of experience in providing physical therapy services in patients’ homes.
- Strong interpersonal and communication skills to effectively interact with patients, families, and caregivers.
- Ability to work independently, multitask, and prioritize numerous responsibilities and duties.
- Bilingual in Spanish a plus.

Physical Requirements:
- Ability to work in patients’ homes, shifting from sitting to standing to walking during every visit.
- Must be able to lift, support and transfer patients up to 50lbs.
- Must be able to perform related physical therapy activities which require lifting, bending, standing, kneeling, and continuous manual dexterity.
