Responsibilities:
- Perform evaluations/assessments to determine the appropriate services needs for the patient referred to Crossroads.
- Will communicate with the referring Physician on assessment findings and goals of care.
- Will work with families and other professionals to ensure the patient receives level of care/services needed.
- Educate patients and their families on disease management, treatment options, and self-care techniques
- Utilize case management principles to ensure continuity of care and appropriate resource utilization
- Great communicator and highly flexible. Must be able to connect and establish positive rapport with patients, families and patient representatives.
- Must be able to work with and coordinate with the Intake Coordinator, Clinical Director and other Hospice team members, referral sources as well as community resources.
Experience:
- Minimum of 1 year of experience in acute care or critical care setting
- Previous experience in hospice or home care preferred.
- Proficiency in conducting comprehensive patient assessments and monitoring vital signs
- Excellent communication skills, both written and verbal, to effectively interact with patients, families, and healthcare team members
- Ability to prioritize tasks, manage time efficiently, and work independently in the field.
