Responsibilities:
- Serve as a primary point of contact for patients, ensuring timely and appropriate
communication throughout their healthcare journey
- Collaborate with Care Navigators, physicians, nurses, and other healthcare providers to
facilitate the coordination of patient care
- Schedule appointments, tests, and procedures, and assist with insurance authorizations and
referrals as needed
- Maintain accurate and up-to-date patient records, documenting all care coordination activities
and interventions
- Provide administrative support to the Care Navigator team, including data entry, report
generation, and other tasks as required
- Participate in ongoing professional development and training to stay current with best
practices in care coordination and patient support
Requirements:
- High school diploma or equivalent; associate's or bachelor's degree in a healthcare-related
field is a plus
- 2+ years of experience in a healthcare setting, preferably in care coordination, case
management, or a related role
- Excellent communication, interpersonal, and organizational skills
- Proficient in electronic health records (EHR) systems and Microsoft Office Suite
