Hospice RN Case Manager Responsibilities:
Holistic Care: Provide comprehensive nursing care to hospice patients, focusing on physical, emotional, and psychological well-being.
Care Planning: Develop and implement individualized care plans that address patient needs, preferences, and goals, ensuring comfort and quality of life.
Family Support: Offer emotional support and education to patients' families, empowering them to navigate the end-of-life process with understanding and grace.
Symptom Management: Monitor and manage symptoms effectively, collaborating with interdisciplinary teams to optimize pain and symptom control.
Communication: Foster open and empathetic communication with patients, families, and the care team, ensuring everyone's concerns are addressed.
Documentation: Maintain accurate and detailed patient records, documenting care plans, interventions, and outcomes according to hospice regulations.
End-of-Life Planning: Assist patients and families in making informed decisions about advance directives, hospice care options, and end-of-life wishes.
Hospice RN Case Manager Qualifications:
RN Licensure: Active registered nurse license in the state, demonstrating competency and compliance with nursing standards.
Hospice Expertise: Extensive knowledge of hospice principles, pain management, and symptom control to provide comprehensive end-of-life care.
Compassionate Nature: Genuine passion for ensuring patients and families experience comfort and dignity during the end-of-life process.

Pay: $90,000.00 - $110,000.00 per year
