The Case Manager/Mental Health Worker provides case management, linkage, and communication services in collaboration with the multidisciplinary treatment team and beneficiaries to WYS’ clients who are in transition or need of resources.
Bilingual Mental Health Worker/Case Manager Responsibilities:
- Provides rehab services to clients directly and indirectly by modeling appropriate behaviors as
- directed by the clinician to resolve agreed upon areas of growth.
- Assists in the intake and assessment and transition processes to help identify needs, coordinate
- mental health services provided by other staff and outside providers.
- Responsible for meeting all regulatory and agency requirements regarding the documentation
- of clinical services delivered.
- Assists in non-billable and billable activities such case management, and rehab services,
- preparing Notice of Action Determination of Benefits and ACCESS log processes as clients
- transition into and out of services at WYS to ensure a high-quality customer care.
- Provide direct client and family mental health rehabilitation services.
- Maintaining up-to-date resources to be provided to families and clients.
- Screen referrals and assist with scheduling intakes.
- Co-facilitate or run groups.
- This position is Direct Service Hours (DSH) producing.
Bilingual Mental Health Worker/Case Manager Minimum Qualifications:
- Bachelor’s degree in a mental health discipline or related field (Counseling, Psychology, Social Work, Human Services).
- Minimum two years’ experience in client-related services in mental health. May substitute 30 semester units in counseling, psychology, or social work for one year of experience.
- Work experience in case management and collaborative partnering preferred.
- Knowledge of working with children and families required.
- Knowledge of community resources, case management process, community outreach and program planning highly preferred.
- Understand various ethnic and socioeconomic backgrounds within a multicultural environment. IC’s
- Ability to navigate Outlook, Word, Excel, Database.
- Bilingual Spanish required.
Salary: from $24.53 hourly
