Position: Clinical Case Manager
Employment Type: Full-time 32-40 hours, non-exempt, salaried and benefitted
Position Summary: The role of the case manager is to undertake assessment, monitoring, symptom management support, goal planning, advocacy and linking of the consumer with rehabilitation and support services.
Duties and Responsibilities:
This person will work full time in a direct service capacity. What follows is a list of duties and expectations for the position.
· Read the Mentis Employee Manual. Implement program goals and objectives.
· Meet regularly with the Program Manager to clarify client needs, to problem solve, and to coordinate services. Submit vacation requests, time sheets and mileage sheets to the Program Manager.
· Attend and participate in monthly agency staff meetings.
· Attend and participate in weekly treatment team meetings to go over progress, challenges, and gain support with how to best work with clients.
· Provide liaison support between clients and the landlords in both housing programs.
· Participate in the screening and selection of clients into both housing programs.
· Facilitate weekly house meetings in SHP and monthly Community Meetings in PHP. The Program Manager will co-facilitate some meetings.
· Maintain active caseload of 8-12 clients.
· Schedule individual SHP client meetings at a minimum of once a month. Coordinate with Program Manager about frequency of individual SHP client meetings. Meet PHP clients in their apartment unit a minimum of once a week.
· Track client appointments, goal updates, and ROI updates for clients.
· Provide 6 months of after-care support to clients that have moved out.
· Provide ongoing guidance, support, advocacy, and resource linkage to address symptom management needs, client acquisition of skills, meet stated goals, maintain stability and placement in housing program.
· Provide support to clients in resolving personal issues or problems as needed. Respond to clients when in an emergency or crisis.
· Meet with SHP clients every six months to review client progress towards achieving identified goals and to review client’s participation in the program.
· Alert Program Manager as soon as possible when a client has an emergency or is in crisis.
· Support clients in working through and resolving conflicts with neighbors (PHP) or housemates (SHP).
· Work collaboratively with others to facilitate consistency and coherency among client’s various service providers, (i.e. doctors/psychiatrists, social workers, Social Security Administration, County Eligibility Department, Vocational Rehabilitation, and some family members, among others.) Arrange case conferences when needed; this should be thought through carefully with the Program Manager.
· Support client’s in applying for housing opportunities.
· Assist clients in move in/move out.
· Write a weekly progress note for each client after each scheduled individual meeting. Document client contact and collaborative contacts.
· Write weekly house meeting note for assigned house (rotates quarterly). Write note for each client, even if client is not present at house meeting.
· Maintain clinical documentation according to the standards of the profession and as instructed by program manager.
· Document identified cases according to MediCal standards when applicable.
· Participate in annual MediCal compliance training.
· Attend provider meetings in the community that are applicable to the client’s treatment, care and wellbeing as requested by Housing Program Manager or the Director of Programs.
· Provide a model of appropriate boundaries and clear communication.

Minimum Qualifications:
· Bachelor’s Degree in a related field, master’s degree in social work, Psychology or Counseling Preferred.
· Experience working with mentally ill adults, preferably in a residential setting.
· Experience in a Medi-Cal funded program is a plus.
Compensation: $27-$30hr.

Actual placement within range will be contingent upon several factors, including but not limited to the candidate's qualifications, education, experience, internal equity, and alignment with market data.
