Job Purpose:
Every Child Pediatrics’ Care Navigation is a collaborative process directed toward patient and family wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation. It is patient/family centered, based on the individual needs and values in partnership with the Every Child Pediatrics team and community service providers. Care Navigators help families overcome barriers by providing knowledge and assistance helping families navigate through complex systems in order to access resources, including financial assistance, translation services, transportation, housing, clothing, food, educational support, and early intervention referrals.
As a special incentive for this unique position, we are offering a $1,000 signing and retention bonus to be split over your first year of employment!
Duties and Responsibilities:
- Assist families in accessing and coordinating care including medical specialty services, concrete community services, developmental assessments and interventions, and school planning
- Help identify patients with special and complex needs and initiate the care plan process including transitions of care
- Develop relationships with patients/families, meeting them in the clinic, via phone and periodically in the community, at school, home, court, etc.
- Serve as the point of contact, advocate and informational resource for patients/families and community partners/payers
- Work closely with medical providers, nurses and behavioral health providers within a team-based approach to care
- Develop, organize and maintain up to date, comprehensive and culturally sensitive resources for patients and families
- Maintain accurate, thorough, timely, appropriate records of all activities and provide data for metrics reports and audits
- Attend community meetings, trainings, committees, etc.
Qualifications:
- Bachelor’s degree (case-management, social work, psychology, advocacy, etc.) or the equivalent with appropriate experience focused on these health care areas: direct patient care, advocacy, public health/education, case management, resource coordination, experience working with medical professionals, reviewing medical records and understanding health and wellness principals
- Will consider an associate degree with additional related experience, education and/or training in aligned areas
- Bilingual strongly preferred (Spanish/English)
- Previous experience working in a medical setting and/or providing case management preferred
- Excellent verbal and written communication skills
- Ability to analyze, problem solve, and demonstrate innovative thinking
