Lead Caregiver will work with Care team members and clients to coordinate comprehensive in-home care. We provide what is known as in-home assistance for elderly, Veterans, the Disabled, and the Developmentally Disabled of all ages, so they can remain independent in their own homes for as long as possible. All services are provided in person, with minimal exclusion. Lead Caregiver primary responsibility requires outreach and engagement of new clients, comprehensive assessment, development of care management plan, coordination of care and family supports. Lead Caregiver position requires excellent customer services skills, people skills, maintain high quality services, driver's license and car insurance. This person is required to build strong relationships with Clients, Client's family, Care Pros, Admin staff, RPNs and our external partners and service providers to ensure clients receive the best care possible.
ESSENTIAL DUTIES & RESPONSIBILITIES
Client Care
- Conduct introductions/ Training assigned.
- Provide on-call coverage for open shifts.
- Collaborate with clients on developing a person-centered, individualized service/care plan.
- Meet with client in their homes and/or community. When an in-person session is not possible, meet with client on the phone or via telehealth as appropriate.
- Organize clients’ care ADLs with other members of the team regarding the care plan.
- Provide support/coaching to Care Pros/Care teams.
- Support Care Pro members in developing skills on the field training.
- Support clients in transitional care services from one setting or level of care to another, including discharges from hospitals and other acute care facilities to home or community-based settings.
- Engages with the family members to the appropriate extent to assess for services and involve in care.
Documentation
- Complete initial assessment and reassessment.
- Complete Client's Care Plan and revise as necessary.
- Maintain proper records on care plan and/or other activity.
- Enter case notes within 24 hours of service.
Other
- Work collaboratively with other provides to ensure appropriate levels of care/support.
- Communicate clients’ needs and preferences in a timely manner to the clients’ multi-disciplinary care team.

- Skills in Microsoft Office, including One Drive, Microsoft Word and Excel.

- The ability to build relationships with caregiver and clients.
- The ability to build partnerships and good relationships with providers and clients.
- Excellent verbal and written communication skills.

- Must work a minimum of one weekend per month (Friday through Monday morning).
WORK ENVIRONMENT
- Field and indoor office environment
- Regularly required to walk or drive to different local sites throughout the day
Position Requirements:
- Proof of negative TB Test
- Registered Home Care Aide (HCA)- If you need it, we'll help!
- Ability to pass a drug screen
- Authorized to work in the US
- BLS/ basic life support- If you need it, we'll help!
- Prior experience in caregiving/development disabilities (required)

Pay: $20.00 -$22.00 per hour
