Position Summary:
The Social Support Staff functions as an active member of the interdisciplinary clinical team. The Social Support Staff provides disease prevention, education and management sessions to the patients of the Health Center and the community at large. The Social Support Staff provides ongoing support and expertise through comprehensive needs assessments, planning, implementation and overall evaluation of individual patient-centered needs. The Social Support Staff plays a critical role in outreach efforts, assisting with outreach clinics, events, and community health initiatives and case management.
Primary Responsibilities:
- Assess individual and community health education needs
- Provide individual and group level health education interventions for Health Center patients
- Coordinate agency health education programs, presentations and health care screenings
- Serve as a health education resource for the larger community
- Conduct needs assessments and develop individualized service plans for patients
- Determine patient eligibility for services and make appropriate referrals
Qualifications:
- Completion of an undergraduate degree, majoring in education, the social, behavioral or biological sciences or a directly related field.
- One year experience in public health education preferred
- Assist with organizing and implementing outreach programs and health clinics for community members who may experience homelessness or need assistance
- Ability to establish and maintain relationships with other members of the interdisciplinary clinical team
- Assist with daily outreach mobile services at all sites and community events
- Ability to establish and maintain professional relationship with patients
- Excellent oral and written communication skills
- Must be able to lift up to 20 pounds
- Must be willing to work in an office setting as well as in the outdoor elements.
Other Duties and Responsibilities
- Develop partnerships with community agencies that best serve the needs of the patient population
- Represent the agency on community committees involving healthcare issues, as deemed necessary
- Maintain any health education/promotion credentialing attained, to include the procurement of appropriate CEUs
- Participate in agency meetings, in-service trainings and workshops, as deemed appropriate
- Setting up table, chairs, tents and backdrop at community events.
- Must be able to drive a mobile clinic van
- Must be comfortable and willing to service a diverse population of patients and clients.
- All other duties as assigned
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
