A Certified Community Health Worker (CCHW) contributes to AHF by blending the mission with care coordination. The CCHW provides preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health.
A CCHW enhances the reputation of AHF by impacting improved member health outcomes and health equity which leads to longer, healthier, and happier lives. The CCHW will facilitate health education, health navigation, screening and non-clinical assessment services, and provide individual support thru advocacy.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders.
The CCHW will work as a part of a close-knit team to address issues that include, but are not limited to, the control and prevention of chronic conditions or infectious diseases; mental health conditions and substance use disorders; need for preventive services, perinatal health conditions; sexual and reproductive health; environmental and climate-sensitive health issues; child health and development; oral health; aging; injury; domestic violence; and violence prevention.
Licenses and Certifications: Current CHW Certification or obtain within 90 days of hire
Managed Care experience preferred.
Bilingual Spanish preferred.
