Job Summary: Primary functions are to provide nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency; and assumes the responsibility for coordination of care.
Job Qualifications:
1. Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
2. Licensure: Texas State licensed as a Registered Nurse
3. Experience: Must be a Registered Nurse. Hospice, or Home Health experience preferred.
4. Skills: Nursing skills as defined as generally accepted standards of practice. Strong communication and interpersonal skills; Sensitivity to issues of loss and grief; Ability to deal effectively with the demands of the job.
5. Transportation: Reliable transportation.
6. Health: CPR certified; TB health screen or TB test/CXR according to policy; Hepatitis B Vaccination Consent/Declination
