Suburban is currently looking for experienced Occupational Therapists, interested in helping primarily elderly consumers with remaining independent and safe in their own homes by conducting in-home independence and safety assessments and administering a new and innovative program called Goal Engagement.
Communities of the highest need currently include Beverly, Topsfield, Ipswich and Gloucester.
Suburban Home Health Care Inc. is an independent, family owned Medicare/Medicaid certified Home Health Agency providing in-home nursing, rehabilitative therapy, and assistance with activities of daily living and personal care 1994.
Occupational Therapists are an important component of the rehabilitative therapy team at Suburban and work in a collaborative, multi-disciplinary environment to deliver appropriate care to our patients.
Goal Engagement is a rapidly growing program based upon the CAPABLE model developed by JHSON and is comprised of a team led by the OTR/L, as well as a nurse and a handyperson. The team works with the consumer to keep them safe and independent in their own home.
Over a period of months, the team and consumer identify self-care goals such as walking to the mailbox, doing laundry, or safely showering. Together the team works to address the goals, (such as home environment repair or modification) empowering seniors to become more independent where they feel most comfortable.
Successful candidates will be trained and certified in the Goal Engagement (CAPABLE) model by Suburban Home Health Care.
