Position Summary:
The Admissions Coordinator's primary responsibility is to meet the needs of Valley's Best Hospice referral sources, patients/families, and staff. The Admissions Coordinator will coordinate the Admissions Process which includes office staff, field staff, outside contacts, H&P, eligibility criteria, DME, transportation, and other services as needed.
Essential Duties and Responsibilities include the following:
- Review patient clinical record and continuing clinical record to assure that eligibility and appropriateness criteria are met as evidenced through documentation and is complete and consistent with care and reflects legal requirements
- Assist Admissions Team in determining terminal diagnosis in consultation with Hospice Physician
- Perform regular collaboration with referral sources, pharmacy, Primary Care Physicians (PCP), PCP offices etc., to establish full clinical picture/dynamic of patient/family
- Coordinate with and schedule staff to ensure timely admission of patient
- Coordinate multiple external resources and contacts to ensure timely admission of patient
- Collaborate with Admission Nurses to ensure full understanding of case
- Perform Hospice Explanation of Benefits to patient and family
- Coordinate and collaborate with Team Manager and Management to assure admission clinical record documentation is complete and reflects all requirements
- All other duties as directed by management
The pay range for this position at commencement of employment is expected to be between $21- 22 per hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Requirements:
Education and/or Experience:
- High School Graduate or Equivalent
- One year experience in healthcare or hospice
- Demonstrates a clear understanding of the hospice philosophy and how each discipline functions. Knowledge of the general principles of nursing care. Demonstrates clear understanding of Medicare Conditions of Participation, California Standards of Hospice Care and other federal and state regulations relating to hospice and palliative care. Demonstrates an understanding of Medicare Managed Care, Medi-Cal coverage and other private insurance coverage
- Medical terminology, multi-tasking, problem solving, and basic computer skills
- Able to successfully interact with varied persons in multiple locations
- Strong interpersonal, customer services, and telephone skills
- Driving required. Valid California Driver's license and proof of automobile insurance
