Case Manager Responsibilities
- Communicate and receive reports from Nurses/LVNs/HHAs, Physical Therapists, Occupational Therapist, and Social Workers.
- Create orders
- Follow up with doctor’s offices
- Follow up with labs for results
- Answer questions and concerns of patients, patient caregivers, and family members.

- Audit of accurate and timely documentation of patient services to reflect the plan of care
- Assess patient and family/caregiver education and information pertinent to diagnoses and plan of care
- Participates in coordination of home health services, appropriately reporting the identified needs for home health interdisciplinary group members. Examples include RN,LVN,HHA, OT, PT, MSW, ST
- Participates in personal, professional growth and development. Also participates in organization’s quality management program
- Maintain a safe and productive working environment following established protocols
- Promotes and supports a teamwork culture and environment internally and externally
- Ensure interactions remain positive and professional with all persons with whom you interact in the course of your work both internally and externally
Qualifications
- Graduate of an accredited register nurse or vocational nursing program
- One year minimum nursing experience. Community home health
- Currently licensed as an RN/LPN/LVN in the State.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Understands philosophy of hospice concept. Also understands needs of the terminally ill.
Job Type: Full-time
Pay: $30.00 - $38.00 per hour
Expected hours: 40 per week
