Responsibilities:
- Provide direct patient care in accordance with the care plan and under the supervision of a registered nurse or licensed practical nurse.
- Assist patients with activities of daily living, including bathing, dressing, grooming, and toileting.
- Monitor and record vital signs, such as blood pressure, temperature, and pulse.
- Assist with mobility and transfers, using proper body mechanics and equipment.
- Prepare meals and assist with feeding if necessary.
- Perform light housekeeping duties to maintain a clean and safe environment for patients.
- Provide companionship and emotional support to patients.
- Document patient care activities and report any changes in condition to the appropriate healthcare professionals.
Requirements:
- High school diploma or equivalent.
- Valid certification as a Home Health Aide or completion of a recognized training program.
- Experience in providing personal care assistance to individuals with disabilities or the elderly is preferred.
- Knowledge of medical terminology and documentation procedures.
- Familiarity with hospice care, memory care, and working with individuals with developmental disabilities is a plus.
- Ability to follow instructions and work independently with minimal supervision.
- Strong communication skills and the ability to interact effectively with patients, families, and healthcare professionals.
- Compassionate, patient, and reliable.
