Job Summary: The Care Coordinator for Early Intervention is responsible for providing comprehensive support and services to children (ages 0-3) who are identified as having or being at risk of developmental delays. The role involves collaborating with families, care providers, and multidisciplinary teams to ensure the child’s developmental needs are met through individualized plans and interventions.
Key Responsibilities:
Service Coordination
- Coordinate and monitor child development and intervention services.
- Develop and implement Individualized Family Service Plans (IFSPs) in collaboration with families and professionals.
- Schedule and conduct regular home visits or telepractice sessions based on family preferences.
Family Support
- Educate and support parents and care providers to promote their child’s development.
- Facilitate weekly Family Support Team meetings.
- Provide recommendations and educational materials to families.
- Explain and implement the Primary Service Provider Coaching Model.
Collaboration and Communication
- Collaborate with the State of Hawaii Department of Health’s Early Intervention Section.
- Coordinate with other service providers based on the child’s needs.
- Maintain clear and concise communication with families and multidisciplinary teams.
- Ensure timely completion of all required documentation and reports.
Assessment and Evaluation
- Assess and evaluate the child’s developmental progress using evidence-based tools.
- Monitor and adjust intervention strategies to achieve desired child outcomes.
Compliance and Productivity
- Adhere to all program guidelines, timelines, and reporting requirements.
- Meet productivity requirements for assigned caseload.
Professional Development
- Stay current with research, trends, and developments in special education and related fields.
- Complete additional duties as assigned by supervisors or management.
Qualifications:
Education
- Bachelor’s degree in Social Work, Social Service, Education, or a related field from an accredited institution.
- Master's in social work preferred
Experience
- Minimum one year of professional social work experience (preferred).
- Experience with early intervention and children with special needs (preferred).
Skills and Abilities:
- Knowledge of developmental disabilities, ADA, and IDEA guidelines.
- Ability to interact effectively within a Primary Service Provider model using peer coaching.
- Strong communication skills, both verbal and written.
- Proficiency in Microsoft Office (Outlook, Word, Excel), Zoom, Microsoft Teams, and related applications.
- Ability to work independently and manage responsibilities without supervision.
- Sensitivity to working with diverse populations and a commitment to cultural competence.
- Strong judgment, flexibility, patience, and discretion.
Work Environment:
- This position involves both office-based work and regular visits to family homes or conducting telepractice sessions.
