**Position Overview:**
We are seeking a compassionate and detail-oriented Assessment Nurse to join our team. The Assessment Nurse will be responsible for conducting initial and ongoing assessments of clients to determine their care needs and developing individualized care plans. This role requires strong clinical skills, excellent communication, and the ability to build trusting relationships with clients and their families.
**Key Responsibilities:**
- Conduct comprehensive assessments of new clients to evaluate their physical, emotional, and social needs.
- Develop and implement personalized care plans in collaboration with the client, their family, and the care team.
- Reassess clients regularly and update care plans as needed to ensure optimal care.
- Provide education and support to clients and their families about health conditions, medications, and care plans.
- Coordinate with healthcare providers, caregivers, and other members of the care team to ensure continuity of care.
- Maintain accurate and detailed documentation of assessments, care plans, and client progress.
- Participate in quality improvement initiatives to enhance service delivery and client satisfaction.
- Stay current with best practices and regulations in home health care.
**Qualifications:**
- Registered Nurse (RN) license in the state of Georgia.
- Minimum of 2 years of nursing experience, preferably in home health, geriatric care, or a related field.
- Strong assessment and clinical skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage multiple tasks efficiently.
- Proficiency with electronic health records (EHR) and Microsoft Office Suite.
- Valid driver’s license and reliable transportation for home visits.
