General Description
The Family Physician provides care to people of all ages from infancy to advanced age, treat chronic conditions, evaluate symptoms, offer preventative care, and let people know when they need to see a specialist. Among other things, this professional offers routine checkups (including well-baby and child visits), health risk assessments, screening tests, healthy lifestyle counseling, and treatment of common chronic conditions. Medical presence aligned to the team-base care model to delegate care; and to provide patient-centered primary, comprehensive and preventive medical care in a community health setting.
Essential Duties and Responsibilities
- Utilizes a team-based care model to share and delegate care responsibilities among multiple caregivers.
- Uses a patient-centered medical home (PCMH) model as a culturally appropriate and team-based approach to deliver high-quality primary care and to coordinate patient care across the health setting.
- Promotes through education, healthy lifestyles on individuals, their families and the community.
- Performs activities aimed at preventing, delaying or reducing diseases and its consequences.
- Establishes presumptive and differential diagnoses of the patient’s health condition and/or its family according to clinical findings and available evidence.
- Develops comprehensive and ongoing treatment plans according to the patient’s health status; including the health prevention and care of acute or chronic conditions based on scientific evidence and the standards of the medical profession.
- Maximizes the patient’s recovery in functional, physical and mental aspects, in order to promote a full and effective participation in the community.
- Collaborates in the design of health education plans according to the needs and characteristics of the community.
- Promotes an increased patient, family and community knowledge on health-related issues.
- Performs a complete medical history, physical and mental examinations on each patient by documenting relevant findings, including comprehensive diagnoses.
- Creates care plans according to the diagnosis and the established risk classification.
- Establishes the type of intervention needed and the possibility of its application by taking into consideration the patient's preferences and the resources available in the health setting.
- Prescribes the type of therapeutic care suitable for the individual and its family with precise and timely indications.
- Guides all actions and behaviors to meet the organizational goals and objectives.
- Maintains an attitude of cooperation and trust among all the relationships with colleagues and other health professionals.
- Delivers an effective communication for the patient, the community, and the care team.
- Knowledgeable with updated skills in the advancement of the medical sciences.
- Applies problem-solving techniques and assumes critical thinking to solve issues related to the individual and collective health.
Supervisory Responsibilities
This position may provide clinical supervision to interns and medical practitioners in their rotations
Minimum Education
Doctor of Medicine (MD) degree from an accredited institution
Residency in from an accredited program (required)
Experience and Qualifications
- Desirable experience in a Team-Based Care model and/or Patient-Centered Medical Home (PCMH) model
- Fully bilingual in both English and Spanish (Written, Oral and Read)
Licenses / Certifications
- Medical License issued by the Puerto Rico Medical Discipline and Licensure Board
- Specialty certification in Family Medicine issued by the Puerto Rico Medical Discipline and Licensure Board
- Active membership in the Puerto Rico College of Physicians and Surgeons
- Federal (DEA) and State (AMSSCA) License for Controlled Substances
- CPR-BLS and PALS Certification
