This person works closely with the facilitator to support positive outcomes for the family. This support person is either a graduate of wraparound himself or herself, or is a family member of a person with complex emotional or behavioral needs (FSP), or was a youth with complex emotional or behavioral needs (YSP).
Works one-on-one and maintains regular frequent contact with the parent(s)/caregiver(s) and/or youth in order to provide information and support throughout the care planning process.
Attends meetings like the Child and Family Teams and IEP meetings to assist the parent(s)/caregiver(s) and/or youth in articulating the youth/family’s strengths, needs, and goals to the Wraparound Facilitator and CFT.
Educates and empowers parents/caregivers and/or youth about how to effectively navigate the child-serving systems for themselves and about the existence of informal/community resources available to them and facilitates the parent’s/caregiver’s and youth’s access to these resources.
Works directly with the youth and family to implement elements of the Wraparound Plan/PCP. Identifies, actively assists the youth and family to obtain, and monitors the delivery of available services including medical, educational, social, therapeutic, or other services; collaborates with the other service providers and state agencies (if involved) on the behalf of the youth and family.
In providing direct support:
- These are identified as action steps on the wraparound plan.
- Can include working with the family and natural supports to strengthen the natural support system
- Preparing the family to attend meetings with school, child welfare, or wrap planning meetings
- Connecting families with resources such as housing programs, recreating programs
- Helping families to reestablish relationships with extended family and natural supports
- Connecting to other families and support groups
- Provide empathy for some families and the ability to understand what they are going through due to shared experiences
- Provide a voice for the family and/or youth until they can do this for themselves (advocacy)
Youth Support Partner:
- Must have lived experience as a child or adolescent who had mental health or substance abuse challenges.
- Experience in navigating the child and family-serving systems.
- Criminal Background check presents no health and safety risk to participants.
- Not listed in the NC Health care Abuse Registry.
- Youth Peer possesses a current/valid driver’s license and an automobile with proof of auto insurance.
- Over 21 years of age up to 32 years of age.
- Bachelor's degree in a human services field from an accredited university and one year of experience working with the target population.
