Summary:
Value Based Care (VBC) Liaison will be responsible to maintain current working knowledge and understanding of value-based programs, in order to provide ongoing and onsite feedback to all applicable parties on program performance. VBC Liaison will provide regular and current insight to providers and clinical operational staff utilizing KPIs, performance scorecards, educational slide decks, and dashboards, while also making recommendations to improve performance gaps and highlight areas of opportunity to ensure the success of our VBC program initiatives here at FCS. VBC Liaison will collaborate with VBC department leadership/contract managers to align educational material and communications to current VBC contract/programs’ goals and initiatives.
PRIMARY TASKS AND RESPONSIBILITIES:

Maintain working knowledge of current value-based care contracts, programs, and initiatives

Responsible for performance management of assigned territory. Support will be given remotely, as well as regular/quarterly in-person regional meetings on current VBC performance status

Will utilize KPIs, scorecards and dashboards to educate providers within assigned territory and provide insight into how performance relates to provider compensation from payers/programs

Required to maintain working relationship with regional operations team(s) to assist with performance management to achieve VBC programs’ success

Responsible for reviewing data to ensure data integrity and accuracy for our VBC programs and providers

VBC Liaison will serve as point of contact between their assigned clinics/regions and VBC department leadership
EDUCATION/CERTIFICATIONS & LICENSES:
-
Bachelor’s degree (B.A.) from four-year college or university; or equivalent related experience
EXPERIENCE:
-
Three (3) to five (5) years cumulative experience in healthcare with clinical or training experience preferred
-
In-depth knowledge of the healthcare industry, EMRs, and quality programs a plus
-
Skilled in Microsoft Office products, such as Excel, Outlook, and PowerPoint
-
Travel: Will be expected to work at any FCS location to help meet FCS business needs
