Under the general supervision of the Partial Hospitalization & Intensive Outpatient Programs manager and as part of a multidisciplinary treatment team, the primary clinician’s duties include but are not limited to providing transitional and comprehensive mental health services including case management, group and individual therapy, family psychotherapy, and crisis intervention, to a caseload of severely mentally ill adults who are high risk for psychiatric hospitalization. Incumbent will complete psychosocial/functional assessments, mental status examinations, comprehensive treatment development, and discharge planning. Coordinate treatment among various agencies and advocate for client to link with needed resources. Work with treatment team and inpatient/outpatient units to facilitate referrals and service. Attend weekly staff and treatment planning meetings.
