Case Manager Licensed Vocational Nurse

SUMMARY:
The LVN is a vital member of the DHS-ODR ICMS team who provides ancillary medication and psychiatric support, medical linkage and case management services to adults experiencing symptoms of mental health, history of homelessness and some with co-occurring disorders. Position requires field or home visits to support members in a community setting. All team members are expected to meet Medi-Cal, Medicare and CHAMP documentation standards set by the agency. The LVN works collaboratively with Step Up’s interdisciplinary teams to provide high quality, effective medication support and care management to members. Care management is broadly defined and can include outreach and engagement to members, engaging members in skilled therapeutic interactions to promote health behaviors, other behavioral health interventions within scope, coordination of care, resource linkages, working with other professionals and organizations in the community to ensure quality of care for members. We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for coordinating care and providing support to patients in various healthcare settings.
Duties:
-MEDICATION SUPPORT AND CARE MANAGEMENT SERVICES:
- Engage with members in a field-based setting, at home and on the phone in a manner that utilizes evidence-based approaches, such as MI, that promote collaboration between the member and his or her health.
- Provide medication support services in the community (members home, field, community agency) as needed.
- Assists with the coordination and linkage of medical, substance and behavioral health resources and ancillary services.
- Develop partnerships with member’s medical and psychiatric care teams and establish ROI’s to streamline services.
- Provide general healthcare education to staff and residents on issues such as diabetes, cholesterol levels and high blood pressure, hepatitis, TB and HIV/AIDS, infection control procedures, management of lice, scabies, etc.
- Coordinate psychiatric care with program or consulting psychiatrist within scope of practice.
- Support Psychiatrist on clinic days and support member during meetings with psychiatric team.
- Provide formal and informal training and support for members on medical conditions including treatments and medications and communicates information and recommendations to the treatment team.
- Provide behavioral health methods to modify unhealthy behaviors, learn new healthy and positive lifestyle behaviors to ensure housing stability and increase recovery skills. This includes education groups related to mental health symptoms and behaviors, diet planning, smoking cessation and exercise.
- Assure medications are prescribed, ordered, and delivered in a timely manner.
- Administer medications and follow up with members to monitor effect.
- Assist members on the self-administration of medications.
- Provide referrals to local resources, including psychiatric and medical care, housing, rehabilitation programs, and drug/alcohol services, socialization activities, providing transportation and accompanying the client when necessary.
- Communicate and collaborate regularly with other treatment providers within the agency or in the community to ensure continuity of care.
DOCUMENTATION AND RECORD KEEPING:
- Complete accurate documentation and become proficient in Electronic Health Record to ensure documentation follows regulatory requirements and accreditation standards.
- Maintain documentation as instructed and within the program timelines.
- Ensure the privacy and security of the PHI as outlined in Step Up’s policies and procedures relating to HIPAA regulations.
- Maintain client files in compliance with Welligent requirements.
- Maintain medication room, medical supplies, and medication storage in compliance with state and federal laws and accreditation standards.
TEAM ENGAGEMENT AND SUPPORT, PROMOTE STEP UP’S CORE VALUES AND COMMUNITY RELATIONSHIPS:
- Collaborate on Member care with other Step Up team participants in weekly systematic case reviews and team meetings.
- Model the highest ethical behavior in relationship with co-workers, supervisor, members, provider, and colleagues in the community.
- Participate in staff meetings, trainings, committee meetings and other staff activities.
- Attend staff meetings, supervision, and trainings as required.
- Support other fellow nursing staff as needed or requested by the Director of Nurses.
SKILLS:
Knowledge of or experience with Medi-Cal, Medicare, and Los Angeles County Department of Mental Health service delivery, documentation requirements, and compliance.
Must have comprehensive knowledge of psychotropic medications, side effects, and administration methods, including the ability to administer prescribed injections safely.
Must follow oral and written medication orders.
Knowledge and familiarization of the use and provision of psychiatric medications, mental health diagnoses such as schizophrenia, bipolar disorder, and depression, as well as co-occurring substance and medical co
conditions (diabetes, hypertension, HIV-AIDS, etc.)
Perform basic physical assessments of individuals, including vital signs, monitor for side effects of psychiatric active medications, and provide information to physicians.
Possess an understanding of and sensitivity to multicultural communities.
Must have extensive knowledge of resources and excellent verbal and written communication skills.
Must be aware of protocol for reporting suspected abuses, neglect, or other dangerous conditions.
QUALIFICATIONS:
California LVN license. The required license must be current.
Conditional, provisional, probationary, or restricted licenses are not accepted.
The candidate must have two or more years of experience in behavioral health, acute medical-surgical, home health, skilled nursing, or direct client services to individuals at risk of or currently without a home, diagnosed with a mental illness, chronic health conditions, or tri-morbidity.
Valid California Driver's license and automobile insurance are mandatory as automobile travel is required throughout Los Angeles County by personal or company vehicle.

SUPPLEMENTAL INFORMATION: Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.
PHYSICAL REQUIREMENTS:
While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
