Near North Health, a longstanding pillar of Chicago's healthcare landscape, is seeking a visionary and compassionate Women’s Health Care Coordinator to join our dedicated team. As Women’s Health Care Coordinator, you will play a pivotal role in our mission to serve primarily low-income uninsured or underinsured residents of Chicago's medically underserved communities. Your experience in assisting providers and guiding patients through their care plans, to ensure their health, will further solidify our position as one of the largest providers of community-based primary care in the city.
Key Responsibilities:

Coordinating Appointments: Primary responsibility is scheduling outside studies and procedures and consults for obstetric patients as the priority, and other reproductive health patients as time allows. This includes sending information needed to the outside institution (notes, copies of labs, etc.)

Learn Various Scheduling Procedures: Learn scheduling procedures for referral institutions and ensure that providers include appropriate information needed to schedule. Stay current with changes in scheduling procedures. Learn electronic/ computer scheduling systems as needed.

Program Representative: Serve as department liaison with referral institutions and offices, attending meetings, collecting information on scheduling procedures and communicating that information to the provider staff as well as keeping DON abreast of any relevant changes.

Patient/Provider Communication: Provide information about scheduled procedures to the patients and providers in a timely fashion so that patient care and convenience is optimized, including all relevant data: location, time, special requirements.
Qualifications:

Proficient in Electronic Health Record, Microsoft Word and Excel

Exceptional organizational and interpersonal skills, with attention to detail required; strong oral/written communication skills are a must

High school Diploma or GED preferred
