Position Description:
The Community Choice Guide II (CCG) facilitate and support clients in the community to ensure the sustainability of the ageing and disabled population. The following is not an all-inclusive list of what a CCG does. CCG help clients by assisting in housing searches, both independent and residential, and all that this entails. CCG also helps customers by providing specialized assistance in stabilizing hoarding behaviors, helping with paperwork, and helping with caregiver searches. Heritage NW Consulting provides services in the Clark and Cowlitz County geographical area.
Duties:
- Contact new clients within 48 hours of the referral being received by Heritage NW Consulting.
- Establishes course of action by exploring options; setting goals with client.
- Meet with client weekly for a minimum of Eight hours of billable time
- Assess the needs of the clients based on the goal summary received from the case manager.
- Recommend Client Training or Professional Support Specialists if needed.
- Establishes course of action by exploring options; setting goals with client.
- Locate and arrange appropriate and accessible housing
- Work with local housing authorities to find available. subsidies and voucher
- Assist with ensuring that all medical resources are available and met
- Provide the client with transition or stabilizing services consistent with protecting and promoting their health and welfare.
- Set up the client’s new home or stabilize the client’s existing community-based setting such as arranging for documentation, arranging any utility hook-ups, or acquisition of furnishings and household goods necessary to establishing or stabilizing their setting.
- Keep accurate records of time worked on each client’s case. Update weekly.
- Maintains record of case by documenting client's situation and client's own actions.
- Update Monthly Activity Records (MARs) weekly
- Complete Purchase Orders and have them approved by the case manager before any purchases are made.
- When items are received, obtain signatures for delivered items.
- Return Purchase order and receipt to Case Manager for reimbursement.
- Monitors planned actions by periodic follow-up with client’s case manager.
- Update case manager weekly on the progress of the client.
- Maintains operations by following policies and procedures; participating in quality reviews; reporting needed changes.
- Complies with federal, state, and local legal requirements by studying existing and new legislation; enforcing adherence to requirements; advising management on needed actions.
- Maintains client confidence and protects operations by keeping information confidential.
- Works as part of a team alongside Professional Support Specialists and Community Transition and Training Specialists
Required Qualifications:
- Bachelor’s degree in social work, psychology, or a related social science degree
- Microsoft 360, Word, Excel, Outlook, and SharePoint
