QUALIFICATIONS
1. Must be a Registered Nurse and hold a current license in the State of Michigan.
2. Must have a degree from an accredited nursing school. BSN preferred.
3. Excellent human relation and networking/communication skills.
4. Knowledge of health provider networks and organizations in five county service area.
5. Possesses and maintains a current CPR certification.
6. Experience with hospice, palliative and end of life care preferred.
7. Must be willing to obtain certification in Hospice & Palliative Care.
8. Maintains a valid Michigan driver's license and has the availability of use on the job.
9. Vehicle must be insured in accordance with the State of Michigan.
SUMMARY OF JOB RESPONSIBILITIES
The Intake Clinical Manager is a role with a strong background in hospice/palliative care and a passion for team leadership and process optimization. This role serves on our leadership team and will assume oversight of the Intake, Admissions, and Live Discharges, along with their corresponding processes. This role will be responsible for adequate staffing to ensure timely patient access to hospice services seven days a week and after hours as needed. Initiates, establish, develops, and maintains professional relationships between hospice and various referral sources, e.g., hospitals, hospitalists, facilities, and independent medical clinics/offices, to ensure a consistent base and flow of patient referrals. Collaborates with the Medical Director on any high acuity referrals or complex admissions. Provides direct supervision of intake department team as evidenced by ongoing ride-along and coaching. Works collaboratively daily with Referral Liaison to ensure timely same-day hospice admissions. Responsible for collaborating closely with the Community Outreach Liaison, Clinical Managers and CEO to ensure growth and customer service objectives are met. Ensures professional handoff of all service recovery concerns to the appropriate Community Liaison or Clinical Manager. Displays an ongoing commitment to excellence in customer service to both internal and external customers and consumers.
ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned)
1. The Intake Clinical Manager will lead, train, and assist as a source and link to the Intake Team and all staff who assist in the referral and admissions process.
2. Leads the end-to-end admissions process, ensuring a seamless transition from initial referral to full admission.
3. Manages the intake team and procedures to ensure that eligible patients receive care.
4. Addresses and resolves any barriers to admissions.
5. Monitor patient and agency growth and ensure profitability to meet or exceed standards.
6. Make certain all patient and family services offered comply with federal, state(s) and local regulatory requirements.
7. The Intake Clinical Manager will correspond and communicate with attending physicians, staff members, and other organizations as needed to coordinate care for the patient and families and eligibility barriers.
8. Collaborate in DPR and live discharges from hospice into community care settings in tandem with their care team.
9. Assess staff efficiency, referral, and admission information to ensure superior support in response to referral requests.
10. Guide and create daily follow-up actions on referrals to ensure timely access to hospice services.
11. Participate in leadership meetings.
12. Takes part in the development and planning of the Intake/Admissions process.
13. Lead daily huddles, along with weekly or other regular meetings with Intake team and supportive groups.
14. Foster relations and promote/provide exceptional customer service and care.
15. Works with Quality and Compliance Manager to define and implement a thorough outline of the referral to admissions process.
16. Contribute to orientation and ongoing education programs to support and educate on the admission processes.
17. Recruit, train and onboard new staff members in department
18. Lead and create training when needed.
19. Primary contact for inpatient referrals, conducts patient/family visits as needed.
20. Chart review from inpatient settings as needed.
21. Conducts admission audits as assigned.
22. Consistently review and optimize the referral, intake, admissions and live discharge processes.
23. Maintains accurate and timely intake statistics. Proficient in identifying trends in referrals/market share via intake statistics and HealthPivots data.
24. This role will be included in the AOC rotation schedule. .

This position has 8-10 direct report responsibilities.
