Senior Cognitive Rehabilitation Specialist, SCRS

In the Clinical Coordinator role, you will be a crucial part to this organization’s commitment to serve others by supervising all clinical and administrative aspects of an individual’s services. You will be responsible for the development and implementation of an appropriate rehabilitation treatment program based on the individual’s needs and input.
- Routinely communicate with the person served to ensure satisfaction with services and active participation in goal development
- Identify effective compensating strategies and communicate this information to the team to ensure a unified clinical approach
- Ensure a functional resource manual is customized for consumers and utilized by all team members
- Provide resources and referrals to outside specialists
- Communicate with the individual’s family/other support systems to encourage their active participation in applying effective strategies
- Collaboratively develop functional and quantitative goals based on ongoing input from the individual served, family (if appropriate), funder, and professionals; communicate the plan for reaching the discharge goal with efficient use of resources
- Clarify the role of each person involved part in the rehabilitation process
- Routinely review and revise rehabilitation goals and activities based on the consumer’s input and response to treatment, and monitor therapy intensity levels
- Ensure that all parties understand the individual’s clinical strengths and needs (includes identifying and educating the support system of the individual)
- Provide coverage/sessions to new clients or clients without staff for up to 6 weeks per individual
- Actively work with Main Line staff to solve staffing/scheduling issues
- Identify and resolve case-specific issues
- Review therapist documentation and initial all approved notes at least monthly
- Provide mentorship, actively promote clinical leadership skills and support the professional development of all staff supervised
- Actively participate in quarterly meetings with the medical consultant, monthly Clinical Coordinator meetings, and Clinical Staff meetings (includes developing the content and presenting the material)
- Address administrative responsibilities including employee evaluations, identification of resources, and communication with other administrative staff
- Complete annual and 3-month employee evaluations and development plans within 60 days of the anniversary date
- Make referrals to appropriate specialists; identify and access community resources
- Ensure the team follows the Code of Ethics and that the individual understands his/her rights and responsibilities
- Ensure compliance with the consumer’s advanced directives
- Maintain daily communication with the office via direct contact, email, and voicemail
Qualifications:
- Minimum of a Bachelor’s degree in one of the Human Services fields
- Minimum of two years of experience providing home and community-based rehabilitation services to individuals with neurological diagnoses
- Must possess certification as a Brain Injury Specialist and maintain membership with the Society for Cognitive Rehabilitation
- Must have a car and a smartphone
- Demonstrate the ability to lead a team and complete the clinical and administrative functions of a Direct Service position
- Ability to mentor and motivate direct reports
- Strong presentation and communication skills
- Demonstrated organization skills and ability to meet deadlines
