WeCareDMV Day Services, Inc., formerly United Cerebral Palsy of Washington, DC & No. VA, Inc., is a non-profit organization that operates a day program for adults with developmental disabilities in the District of Columbia.
The primary responsibility of the QIDP is to support people with disabilities and special needs to achieve their personal goals. The QIDP Coordinator will be responsible for overseeing the development and implementation of the ISP (Individual Service Plan) for each person and will take a lead advocacy role to ensure persons receive appropriate care, services, equipment, and successful community integration. The QIDP Coordinator will work with the Day Program Manager to monitor Direct Support Professional (DSP) daily documentation. The QIDP achieves this through inspirational leadership and positive teamwork.
This position is responsible for completing the following duties in accordance with WeCareDMV Day Services, Inc. (WCDMV)’s policies and procedures in coordination with Medicaid and the Department on Disability Services (DDS) directives.
JOB RESPONSIBILITIES:
1. Demonstrate respect for people with disabilities and their families or advocates.
2. Demonstrate and practices the agency’s goals and mission.
3. Practices adherence to DC Department of Disability Services regulations.
4. Works with DSPs to provide training, modeling, coaching, and support to assure compliance with daily documentation and individual service delivery.
5. As part of the Interdisciplinary Team, the QIDP performs as a lead advocate and primary liaison for WCDMV with outside agencies to ensure that the individuals receive appropriate care, supports, services, equipment, and successful community integration.
6. Provides follow up to recommendations regarding coordination of services and support made from outside agencies (DDS, PCR Audits, and other regulatory agencies.
7. Coordinate meetings with WCDMV’s DSP and Day Program Manager to develop person-centered programming objectives and community inclusion activities for the individuals served.
8. Provides an initial Individual Program Plan or Community Integration Plan for each new client within 30 days.
9. Develop, generate, and distribute the ISP documents (Individual Program Plan, Community Integration Plan, Positive Personal Profile, Person-Centered Tools, Job Search, Activity Calendars, etc.) based on person centered and interdisciplinary team planning based on the individual’s needs and interests.
10. Completes the quarterly reports for everyone served in the program to determine the progress of the goals and data collection to ensure that the goals are measured and documented in the daily summaries.
11. Ensure prompt submission of all ISP, Quarterly, On-going Community Integration, Court, and other reports prior to noted deadlines for submission.
12. Submit reports for internal review prior to uploading to DDS’s Mission Command Information Systems (MCIS).
13. Represent WCDMV via in person or by providing documentation at court hearings.
14. Attend and participate in Intake and Discharge meetings, Special Reviews, Human Rights Committee (HRC), Behavior Management Committee (BMC), Incident Management Committee (IMC), Quality Assurance Team (QAT) and Administrative Interdisciplinary Team (Admin IDT) meetings.
15. Participate in WCDMV incident management activities pertaining to the individuals served by documenting and reporting incidents as needed and assisting with analyzing monthly trending and tracking data and follows up on corrective and preventive actions.
16. Conduct weekly, monthly, and quarterly audits to assure all program books, medical books, progress notes, calendars, and other required documentations are current and complete.
17. Assist in the development and revisions of program policies, procedures, and protocol to remain current with DDS/DDA policy updates as they occur.
18. Maintains positive, respectful, and professional interactions with individuals, staff members, DDS and all other customers at all times.
19. Regularly observes individuals’ health status and immediately reports all unusual health conditions to WCDMV nurse.
20. Supports Saturday Services Program and Community Inclusion activities as needed or requested.
21. May assist DSPs with direct support services to individuals including mealtime assistance, when needed. Perform other duties as required in support of WCDMV.
Candidate must have a Bachelor's Degree in a Human Services or related field, a minimum of two (2) years of experience working with individuals with intellectual or developmental disabilities with demonstrated ability to fulfill job responsibilities, and one (1) year of experience in providing supervision and developing individualized program plans and/or coordinating services. Must possess a valid driver's license and good driving record and pass a criminal background check.
