Aledade is seeking a CHC Program Manager to support the development and advancement of internal and external Community Health Center-oriented resources to drive ACO performance via a CHC Center of Excellence. This position requires experience working with Federally Qualified Health Centers and understanding the basics of value-based care.
On a day-to-day basis, this role will support management of projects driven by the CHC team, training and development of internal staff, build processes to support stakeholder needs in the CHC space including some data analysis, process mapping, support the management of external consultants, and other administrative/operational duties to support CHC performance.
This role works with both internal and external partners, necessitating strong communication and stakeholder engagement skills. We are flexible with respect to geographic location, and the ideal candidate will be comfortable working remotely/work from home within the US, with periodic travel required.

Identifying the optimal approach for a given project including defining roles, stakeholder involvement, risks, dependencies, outcomes, reviewing and approving processes, anticipating milestone dates, and adjusting throughout the course of the project through the lens of FQHC needs

Developing and maintaining program documentation, such as project plans, schedules, communications plans, etc. Identifying resource constraints, risks, and conflicts with potential impact to project deliverables. Providing successful program management while balancing multiple priorities

Collaborating with internal and external stakeholders and cross-functional leads to manage program scope, plan resources, coordinate schedules, and facilitate communication

Sorting and organizing information - from Tableau, from stakeholder questions, work that was done in the past - to identify program paths forward

Administrative tasks as needed to drive the program forward, such as scheduling meetings, note taking, creating spaces to share information with others

Bachelor’s degree and/or 7+ years of healthcare work experience

Experience or exposure working with FQHCs

2+ years of progressive value-based care
