We are looking for an experienced LVN who is familiar with the HCS and TXHML Programs.
This is a nursing case management position including clients living in a group home setting. The LVN works with the RN, Host home providers, and direct care staff to ensure the health and safety of each client. The focus of this position is monitoring the client's use of medications and monitoring health data and information and assisting in the health care aspects of the client's daily needs. The successful candidate will be willing to travel in the Central Texas Area for face to face visits as needed, participate in annual planning conferences and be available to the families on the case load as needed.
